Pernaco is Perniciacum, a station on the road from Bavay to Tongeren in Belgium, most likely at Braives in the province of Liège. The Roman register of routes gives the full name, Perniciacum, which the map shortens. Older research proposed a long list of villages, from Perwez to Avennes and Montenaken. The map's huge figure for the stretch from Geminiacum cannot be right, and the site itself was in use from the Roman period into late antiquity.
